Ok I asked for some more information, but let me give it a try to help you with what I have here.
The two holes are for water to drain out of the over flow of the sink. If you look at the sink on the front side you may have to feel but there is one hole or up to three little ones. They take off water when someone walks away with the faucet on and they it goes down the drain and not on the floor.
Now if the sink your putting this in is a steel sink that does not have an over flow it will leak and you need an other drain system that is made for a sink with no over flow.
Places like manufactored homes, boats, campers would be where you would find a sink that does not have a over flow. Now with the over flow out if the sink does is not about1 inch thick.

The drain stop rod that goes through the fixture and would hang in your vanity should have a piece that bolts to it with holes in it. you need to pick the hole to go over the shaft taht sticks out of the drain pipe, That controls the stopper, taht will close the stopper when pulled up and, will open stopper enough to let water drain. You use the "v" shaped piece with the 2 holes in it to attach the two together. You have to position the v piece over both sides of the plunger piece and then squeeze and put the whole thing over the shaft of the pluger actuator. If you type, "how to install a faucett" in your search window, you can pick a video on how to install a "bathroom faucett" that will give you pictures. It's really hard to try to explain.
the small hole underneath handle where you put your hand has a small set screw(alan wrench)-should be able to see hole,loosen and wiggle handle should pop off, spin off packing underneath and pop out stem make sure water is off - whole new stem is cheap
pop the cap off top of handle with steak knife, screw is underneath,unless yours is like the one pictured,then its an allan screw from the back.you had one when it was installed and,these can be really stuck
Probably what you need to do is change the cartridge. You will need a Moen cartridge puller to help with removal and reinstallation. You can get these and the cartridges at most hardware stores. Shut Off Water first, remove set screw that secures handle and carefully remove handle. You may have to tap lightly on rim of handle to free it. There is a retaining clip that holds cartridge in place, it will have a small round hole on top. Carefully remove clip using neddle nose pliers or perhaps ice pick inserted in hole to remove clip.Install puller as per instructions and tighten nut to remove cartridge. Install new cartridge andclip. put handle back on and install screw. I need to back up here and say that screw is normally located under moen insignia in middle of handle. Pry cover off with screw driver or knife blade to access screw. After you have cartridge, retaing clip and handle reinstalled turn water back on and check for leaks. Hope this helps. Thanks.
If you have a 3" hole I would definately say that is a special order faucet, But if 3" is a typo you can get what are called "cockhole covers" to cover the hole you're not using. What i usually do is, find a faucet that will use the 3 holes and install a soap dispenser, filtered water spigot, or a instant hot water spigot to fill the fourth hole.
If 3" is not a typo you might try to contact a granite countertop co. or call Jacuzzi and install a larger plate that will cover large holes and have new (smaller) holes drilled for a standard faucet
