go to an electrical wholeshop shop that caters for the trade and they will have it if such a thing is available
hard ware shops may have but as it is a specialized fitting , I am thinking only the wholesale out lets will carry it

Sometimes the base screws, on brand new tvs, come in the 4 holes in back for mounting the tv on the wall.

Buy an inexpensive screw hole size template (used for inserting screws of unknown size into it, and reading what size hole it fits into, and then going to buy that screw) at Home Depot (HD) or Loews. If there is room to lay the template over one of the holes, you can eyeball which hole on the template is about the same diameter of the screw hole on the base. Then go to the store and look for that size screw (machine screw Vs wood screw), and purchase one of each of three sizes (the one that appears to be the one, and one that is the next size down, and one that is one size larger. Then see which one fits in the base (this will be a bit of trial an error). This should work until the right size is identified.
