Run fault codes and check for faulty cam/crank position sensor. This is indicated by what you have said. Check battery terminals are clean and tight and check engine immobiliser
Here is the update. finally tested the coil pick up (located inside the distributor) with an OHM meter, now this was a brand new part that was bad from the start returned it got it exchanged and voila my Truck is running. Thanks for your help. I guess it just goes to show ya part can be bad from stock.

My problem was solved when I repaired my 4 wd activation dash switch. The switch was contacting on 2WD & 4HI at the same time. The front differential activation is vacuum activated possibly creating a tug of war that didn't actually engage it but drained off vacuum from the engine? A theory at best but stumbbled across it because I knew the switch wasn't working properly I repaired it and low and behold rough idle disappeared and all is good for 3 weeks running.

Did you check for codes. It won't matter what you change if your computer failed. You should of payed the diag fee. You would of saved alot of money.

when the engine starts the O2 sensor volts should be around .5 volts or 500mv so the 450mv is correct now i don't know wither your next statement is 115mv of 1150 mv it makes a big difference, so get back with me and i will see what i can do for you.

Check the plug connector to the ignition module, it could of disconnected while you were making repairs. Check for spark at the plugs.
Engine stalling issue at low speeds
This problem is commonly caused by a dirty automatic idle speed control valve and throttle valve. Buy a can of throttle valve cleaner from NAPA or Carquest (made by CRC chemicals) and spray it into the air intake while the engine is running, use up about 1/2 the can, engine will try to stall hold the speed up, shut it down and let it soak for 30 minutes, restart and blow out the remaining fluid, shut it down and disconnect the negative battery cable for 5 Min's to reset the base idle control
