Engine starts but stalls.
gunk/dirty gas in system (let tank run down too low).
used gas treatment- carb/fuel injector cleaner
ran for a while (100 miles), then stalled again.
replaced fuel filter.
used carb/fuel injector cleaner spray.
still the same problem
could it be a clogged fuel injector?
is there another fuel filter somewhere?
Best do a fuel pressure test. Should have chg. fuel filter from the get go. The only other filter is on the end of the fuel pump in tank. Bad gas and dirty gas will plug up fuel filter, hense burn pump up.
The more i think about it, i don't think its totally fuel related sense you did not mention you were getting plenty of fuel i was under the impression it was starving for fuel. I think (witch sometimes is dangerious) you might have an electrical problem to, now you said you drove it 100 mi. and then it stalled, did it start right back up or did you have to waite a while like 5 or 10 min.or more? What i'am thinking is the distributor maduel could be bad, they been known to work alright when cold but when it gets hot they don't. You can take it out and have it chk. at any major auto parts store.

Here are the most common causes of idle surge, stalls at stops, slow idle speed, erratic idle speed, rough idle and engine hesitation (and other problems), it is in most cases the idle speed control air-bypass valve and or throttle valve and upper intake, these area's get full of gunk and combustion residue over the miles and cause idle issues (stalls, low idle) like yours, Get a can of intake cleaner from any local parts store, not carb spray, intake cleaner, it is made by a company called CRC, remove the air intake hose to the engine, hold the idle high so the engine won't stall, then spray the can of cleaner into the intake while keeping the engine running, use at least 1/2 the can, shut down the engine and disconnect the battery for 5 minutes, then restart and complete a number of mixed driving cycles, town, freeway, stop and go etc., after a few days the problem will go away as the system will relearn to the clean intake

Could there be a problem with the throttle body?
When changing the fuel filter, we turned the ignition on and plenty of fuel came out of the fuel filter so we think it's okay.
No, we did not have to wait 5 or 10 minutes. Once it stalled, it would start again. But every time we started it, it wanted to stall again right away. To keep it running, we'd have to constantly pump the gas pedal, and the van would buck, lurch and run very roughly. Then it would stall again after a minute or two. Then the same thing would happen... we'd start it, run it like that for a while, then it would stall again. When it runs, it backfires and runs very, very roughly. On one side of the throttle body we are getting a fine mist/spray of fuel. On the other side of the throttle body we are getting a slower supply, not a mist. We are leaning to thinking it's either a bad fuel injector or a weak fuel pump. What do you think?
