You should bring it in somewhere to get repaired. There are videos on youtube that might tell you how to fix it, but then you'll void your warranty.

The disc reading problem is caused by the laser coming away from where it needs to be. You can either send it in and they will fix it for you or you can repair it yourself using a good repair guide.

You can go on playstation's website and trying logging on your PSN from there, and if you forgot your password there is a "forgot my password" link

I would check with the Sony PS3 website and also whoever makes Call of Duty and see if there's a bug or fix for the software.

try shutting your system down and booting again. this has happened a few times to me.

have you tried keeping your finger on the on button for about 3 seconds until you hear a couple of beeps? This allows you to change the video input manually from scart/hd etc...
worth a try.....
