My sincere apologies for the delayed response.
I am personally going back to answer your unanswered questions.
I have only been a free contributor to this site for the past 4-months.

I do not know if you still need this information, but I am going ahead and answering it anyway.

CLICK on the following LINK. It has several Belt Diagrams, one which will match your specific engine and options (AC/No AC, Power Steering, .etc) that you will need.

Regarding HOW:
Standard Rule to Removal the Serpentine Belt:
1. Find the Tensioner(s). (See the Diagrams on Link)
2. Using a Serpentine Belt Tensioner Tool or Wrench or Ratchet Tool: Rotate/Move the Tensioner Pulley/Arm away from contact with the belt and towards the area where the belt is not in contact with the Tensioner Pulley. The Tensioner is spring loaded, and is hard to rotate/move.
3. Remove the loosened belt off one of the other more easily accessed Pulleys.
Standard Rules for Installing the Serpentine Belt:
a. Pick a Pulley that is most easily accessed. This will usually be on top. This will be the last Pulley that the belt will go on.
b. Using the Diagram: Install the new Serpentine Belt on the remainder of the Pulleys....over, under, left right etc.
c. Using a Serpentine Belt Tensioner Tool or Wrench or Ratchet Tool: Rotate/Move the Tensioner Pulley/Arm "away" from Belt contact area on the Tensioner. This spring is pretty hard and with a new belt, it will be even harder to install. Rotate/Move this to as-close to the maximum ("open") allowed inorder to have enough slack in the belt to get it up and over the last pulley.
d. While keeping the Tensioner Pulley/Arm "open" Using your other hand - Pull the Belt up and over the Last remaining Pulley.
e. Before releasing the pressure on the Tensioner, visually inspect the remainder Pulleys and the Belts' Positioning on them.

What is the average life of a serpentine belt?
It varies from vehicle to vehicle but you could ask the dealer---my view has always been to keep an eye on the belt for cracks or fraying anywhere---they are not that hard to replace and while costs will vary (on mine found a OEM one new for 26 dollars and same belt at local parts place was 52---same brand and part number.

It is better to replace it and have the tension unit that tightens most of these inspected than have it go when you are driving down the road.

In my case I keep the old belt rolled up and stowed in the vehicle in case the new one fails.

The pulley youll need to replace is called a pulley tensioner,it comes with a automatic tensioner and a pulley all in one unit, when this tensioner wears the drive belt can no longer maintain a correct tension, when tensions are loose a belt noise develops as high RPM are achieved you develop a squeeling noise even at start up. Failure to replace a worn tension developts a glaze on the drive belt thus wearing it out quicker,by all practical means if you reuse the same belt the rotation must be followed,drive belt being rubber in nature developts conicity and fail if rotation is not maintain,so it always a good idea to replace the drive belt when you replace the pulley tensioner

Replace belts/how to?
Belts are the easiest thing to replace with a wrench, and a prybar.
If you have a serpentine type belt, use a 1/2" breaker bar to relieve tension on the belt, noting the routing of the belt.

If you have a v-belt, there will be an accessory such as an alternator that has adjustment bolts on it to tighten or loosen the belt.

BELT ADJUSTMENT
  1. Loosen the tension adjustment and mounting bolts.
  2. Move the pump toward or away from the engine so the belt deflects 1⁄4 – 1⁄2 in. (6–13mm) midway between the idler pulley and the pump pulley under moderate thumb pressure.
  3. Tighten the bolts and recheck the tension adjustment.
The following describes varioius types of belts and their inspection:
Belts
INSPECTION Check the drive belts for glazing, cracks, fraying, wear and tension every 6,000 miles (9,662 km). A glazed belt will be perfectly smooth from slippage, while a good belt will have a slight texture of fabric visible. Cracks will usually start at the inner edge of the belt and run outward. All worn or damaged drive belts should be replaced immediately. It is best to replace all drive belts at one time, as a preventive maintenance measure, during this service operation. It is recommended that the belts be replaced every 24 months or 24,000 miles (38,647 km). Belt deflection at the midpoint of the longest span between pulleys should not be more than 7⁄16 in. (11mm) with 22 lbs. (10 kg) of pressure applied to the belt when engine is cold. Fig. 1: There are typically 3 types of accessory drive belts found on vehicles today tccs1218.gif
