It sounds like it has been poorly fitted, check the centre of the pully wheel and se if it sat correctly on the shaft end. if you can loosen the belt and tap the centre of the wheel to see if it re-aligns, be careful driving it, in fact I would not drive it at all until it looked correct.
I had a similar fault on another car one of the bolts hokding the pully wheel had came loose and fell out there was 3 bolts on this car, not sure on yours but do chcek they are all tight and be sure to tighten them evenly half a turn each then move to the next etc etc.

NO, the crank pulley, or harmonic balancer is rubber mounted, it will come apart, and is a fairly expensive item, $100 at the junk yard. Get it fixed before it flies off the car. Hope this helps.

Yes for the tensioner and maybe for the crankshaft. If the crankshaft bolt is loose, the pulley could work its way off the crank.
What you need to do is start fresh with the tensioner. A new one will insure that the pulley mounted on it is the correct one. Be very careful about the bolts going through it as some may vary in length having bad consequences.
Even if you have to go to the Dealer, find out about bolt lengths. Then look at the back of the tensioner bracket. For strength some brackets have raised surfaces which are suppose to match indentations in the mounting surface. These 2 irregular shapes are suppose to mesh and prevent vibration from unseating the tensioner.
You may be able to find a tensioner in the Salvage yard with its bolts intact. Label position of each bolt removed. They may be different by 1/2 inch or less. Also looked for spacers or washers that would help align the part.
Make sure that the belt you have is not stretched.
