I also did full default reset from admin webpage and from maintenace lcd menu. I also disconnected printer from power AC for 1 hour. Any other ideas ?
Sometimes if there was a paper jam and someone removed it without getting all of the paper, then there could be a small piece of paper still in the way of the sensor. If you have any compressed air it might help to blow it out of the sensor area. I have had issues with that in the past as well, so I opened up the area it said it was jammed - gave it a good going over with the can of compresses air, and it seemed to clear out the jam.

CVO UP_LU_FU shows 1.L 2.* 3.H 4.* where H is close. 1 is Cover-open switch and 3 is rear cover. So system is reading from sensor 3, but is not reading state of sensor 1. This one made me crazy, but I finally figured it out. Turn the machine around so that you are facing the back. Open the back cover. Pull down the second smaller roller cover inside the machine. If you look closely at the second smaller cover, on the right hand side (closer the the inside of the machine), is a very thin round piece of plastic that is only half the length of the smaller cover. There are 2 thin plastic tabs attached to either end of this round piece of plastic. Gently shift this over so that the 2 plastic tabs can now go into the space provided for them. I had to move mine over only 1/4 inch.

We had this problem. It turned out there was actually a piece of paper stuck in the printer! It wasn't so easy to see it from the rear.
We removed the paper from the front by taking out the ink cartridge first. Turned the printer back on and everything was fine.

There is a little cover there that has a sensor flag on it. It is most likely in a jammed position. If you are able to carefully take the cover out, you can bend it slightly, readjust the sensor flag, and reinstall it.
