* your best bet would be to remove lock set and take it down to your nearest Home Depot, Lowe's, City mill, Ace Hardware, etc. It's a pretty common lock brand. The store key maker, door hardware section should be able to help.
* I've found it's best to replace the lock set as these combo locks only last about a year in heavy traffic areas. Especially if it's exposed to weather.
* Constant opening and closing shakes things up real good. Lots of small wires and plastic parts break all the time.
* Key pads wear out fast. Especially if the code is not changed every month.

Open the right/rear door. Pull the rubber weatherstrip back from the door opening. Fold the right/rear seat back down into a locked position, in order to access to the right/rear trim panel. Carefully, pry the right/rear upper trim panel housing the right/rear seat belt height adjuster. Now carefully pry back the lower trim panel taking care now to break it, exposing the vehicle security module with three connectors plugged into it, coloured green, blue, and grey. Look on the label that has the part number which should end in EB. There are FIVE numbers to the right of that number. Those five numbers are your keyless entry keypad code.

You can try programming it but first make sure the battery is good. Locate the smart/learn button on the rear of the motor unit near where all those white wires go. Press and immediately release this and then walk over to your keypad and key in any 4 digit code you like and press enter.
look on the sticker located inside the outer panelo on the lock. you are going to need to disassemble the unit but it is in there.
