The number od R's in front of the model number (which starts with the letter K ) is the number of times that the machine has been returned to the factory for over hall. next your machine was built / manufactured in Greeneville Ohio in 2002 sometime in April. You have possibly blown the fuse gear. Need a bit more info on the noise. If you turn it on slow and help the planetary does it try go get itself going?
This is a kitchen aid. BUT!!!! it is an international model that was sold in countries with 220 volts. Look underneath the mixer for a white tag it should have a model number and a serial number, if you can get us those numbers then we can help with the parts. Hope this helps.
It is correct that the knob is not separate from the shaft it is attached to. If they are obsolete then hop into your local Kitchen Aid retailer and see if your missing knob looks same or similar to a current model, if so order for a current model, lever the knob off the shaft (carefully) and tap it onto your bare lever. the rotating plate needs to be pulled of and clean the grease out and only put two teaspoon dabs on ether end. to pull it of you will need to push the pin in the middle out and just lightly tap it back in.

im sorry to hear what is happening with your mixer. I do not know any places around that area. but i would recommend service. It sounds to me as if it could possibly be the motor assembly not lubricated properly or it could be a power switch going out. either way will be serious to the operation of your mixer. i am not to familiar with that model but im sure the parts can be redily available
