I've tweaked the display settings to extend to 2nd monitor - but still nothing displaying on the tv, why?
Try lowering screen resolution on TV. There is a chance your video card can't handle HD like 1080 dpi or higher. Plus make sure TV input is set to VGA.

Make sure your TV does indeed support the resolutions you are trying to use. Some low quality HDMI cables can also be troublesome in higher resolution so try one you are sure to work.
Regarding to the desktop only being extended when using the RGB cables, try bringing up the desktop properties window, select the TV as the primary monitor and select "Use this monitor as Primary monitor". You might have to drag the windows Taskbar over to the other monitor, just rightclick the taskbar, uncheck "Lock the Taskbar" and drag it over to the other monitor.

I can help with getting your desktop onto your TV, but sound from your computer through HDMI can be much trickier.
If you only see a background picture on your TV, then your desktop has been either "extended" or "stretched" onto it. It is essentially a second independent monitor and you can drag applications over to it to see them.
To view your desktop and start menu on your TV, you need to run ati catalyst and "clone" your TV with your computers monitor. Do this by right clicking the picture of the second monitor in the catalyst display manager and choosing "clone". Now your tv and your monitor act as one.
Although i tried 3 different ati cards with hdmi outs that claimed to pump audio through hdmi, none of them worked for my HP PC. You will need a grey widget adaptor between your computer and the hdmi cable. In the end though, there's a conflict between my sound card and ATI's HDMI audio that cannot be resolved. So I run a digital SPDIF signal instead through an rca cable and get 5.1 dolby at my amp that way. Good luck.

Had a similar problem, some video "jitter" and then the screen washes out with backlight showing. ONLY HAPPENS when under battery power.
Cause: a recent reinstall of XP SP3 realized a newer video driver, intel integrated 945gm, than was on the Dell support site. This newer driver is problematic.
Download and install the Dell support site's OLDER video driver for the D620. A new battery won't help, only the older driver.
