Yep...I have the same problem, along with the dial tone studdering. This unit has not been heavly used, so it should NOT have these kinds of failures...what's up with this???

Clean the scanner head and glass

Removing the scanner glass

General
The Automatic Document Feeder (ADF) will need to be removed before the glass can be removed. You will also need a T10 Torx head driver approximately 6" long. Driver handles that accept removable bits may be too large in diameter for this particular job.

Removing the automatic document Feeder (ADF)
1. Hinge the ADF upward as if you are going to manually scan a document.
2. In the hinge area there is two black pull latches. Pull them up and lift the ADF straight up to remove it. Some side-to-side rocking may be necessary before it will disengage.

Removing the Scanner Glass
1. Remove the four T10 Torx head screws around the perimeter of glass, i.e. two in the ADF hinge area and one on the right and left.
2. Remove the Control Panel overlay. It pries off.
3. Open the printer as if you are going to access the cartridges and disengage the support legs. Two latches at the base of each leg must be pried outward with a slot style screwdriver to disconnect them. The scanner assembly will now "flop" freely, so steady it while removing the four screws on the underside.

Note: The screw in the front is hidden under the piece that you grasp to gain cartridge access. Two latches must be released to remove it. The screw can now be seen using a flashlight but may be obscured by some wires.

Clean the underside of the glass, giving particular attention to the white strip. Then clean the scanner bulb (gently, its thin glass) and the mirror(s) with a cotton swab sparingly moistened with Windex or a similar cleaner.

Hi lore, From HP's support site: Issue One of the following messages displays on the control panel of the all-in-one: * Warming up. * Scanner failure, Power off and on. NOTE: Before doing any troubleshooting, try making a test copy of any document by pressing the Start Copy (black or color) button. If this copy is successful, the scanner mechanism is functioning correctly.Follow the troubleshooting steps below only if an error message displays when trying to make this copy. Solution These errors usually occur when trying to scan, to make copies, or during the product initialization. NOTE: Extreme temperatures can affect the operation of the All-in-One. The recommended operating conditions for the HP All-in-One are listed in the User Guide provided with the unit. Step one: Check the power connections 1. Make sure that the unit is connected directly to a grounded wall outlet and not plugged in to a power strip. Perform a new color copy by pressing the Start Copy Color button on the front panel. 2. If the error message does not go away, temporarily move the unit to a different room or outlet and perform a color copy by pressing the Start Copy Color button on the front panel. 3. If your product uses an external power module, check the power module part number against the part number listed in the User Guide to ensure the proper power module and cord are being used. Step two: Perform a power cycle 1. Turn the unit off. 2. Unplug the power cord from the unit. 3. Wait 30 seconds, and then re-connect the power cord to the unit 4. Turn the unit on. 5. Perform a color copy by pressing the Start Copy Color button on the front panel. If the error still occurs, proceed to the next step. Step three: Reset the product 1. Unplug the power cord from the product. 2. Press and hold the appropriate button combination for your product. The appropriate reset combination for your model can be located by performing a search for your specific model and the word reset. 3. Follow the instructions to reset your product. 4. Perform a color copy of any document by pressing the Start Copy Color button on the front panel. Step four: Check the scanner light 1. Perform a color copy by pressing the Start Copy Color button on the front panel. Slightly lift the scanner lid to see if the scanner light is visible during the copy. Use caution to not look directly at the light. It is not necessary to completely open the lid to see the light. 2. If the scanner mechanism moves across the scan bed, but the light is not lit, and all of the above steps have been performed, proceed to the next step. Step five: Service the product If you have completed all of the steps listed above and are still experiencing a problem, service your product. If you run the first test and it pases let me know and I'll see what else we can do to resolve your problem. Hope this helps, Dude

How about posting a real solution instead of these silly, canned helpdesk answers that everybody already knows to do and has already done before they look for help elsewhere. Service the unit you say? Getting one of these pieces of crap fixed probably costs more than a new and better all-in-one scanner/fax/copier/printer. Do yourself a favor and take the thing to the range and fill it full of 12 gauge slugs. Then go buy a new one at half the cost.

When it works..is the paper being picked up from the ADF...on top of the printer.
I noted, I can scan from the flat bed but not from the ADF (Auto Document Feeder)
I have one on the bench, scanner error comes up when feeding from ADF. The ADF is not working.
I suspect this could be the problem.

I would agree with "assrot" on this one. The "canned" HP answer was just to make HP look good for this piece of **** series (D125,135,145,155) that they put out. This was not my favorite machine to work on. "Scanner Failure" was a very common problem as well as "IDS Failure" on these machine. Scanner failure is either the main board the scanner plugs in to or its the scanner itself not all that other BS they want you to try. Bottomline problem is not economical to fix...good luck even finding a replacment part.

Go into Device Manager and double click on USB Print Support. In Driver tab, click uninstall. After uninstall, power reset the computer. Displays " Scanner Failure"

I've seen a similar issue on other HP All-In-Ones. I had to contact support, but all they do is make sure you try connecting the printer to the wall outlet instead of a surge supressor, then a different wall outlet, then reset to factory defaults. After that didn't work, they submitted an replacement order.

Hope this helps
