Forgot pin and now need PUK
The PUK code is a code associated with the SIM card and not with the phone.
You
have only two ways to get rid of the PUK code. Either you replace the
SIM card , or you call phone provider that issued the sim cadr (eg.
At&T, Virgin, Tmobile , Vodafone, Telecom etc.)
PUK means "personal unlocking key" .
The PUK code is asked if you enter a wrong pin 3 times.
You need to call the phone company that released the SIM card, they can retrieve the PUK code from a database for free.
In your case you need to contact your SIM card provider technical support to unlock the SIM.
The PIN and PUK are related to the SIM card, not to the phone.
If you replace the SIM card with a different one, then the phone will not ask for the PUK anymore.

Do not enter any code into your phone. The PUK code is a code that your service provider has and not one that you would know or have in your manual. If your an AT&T customer they do provide PUK on their website. If you have entered in a code and the phone is now saying PUK blocked you'll have to go to store to purchase a new SIM card, you may want to call your service provider as well, they may be able to give you a credit for the new card. Hope this helped.
How much would it cost...
In repairs probably more than the phone out right. The cell phone repair shop near me charges 80 dollars for a speaker replacement, 50 for a charger port. I am not sure about the memory card reader or the volume keys but it sounds like you would be better off getting a whole new board and just keeping your housings, faceplates, and lcd.

Ciphering is mainly caused due to old sim cards,remove your sim and
reinsert it.If the problem still persists,go to the customer care
center of the operator and ask for new sim with the same number ,you
would probably get this for free .Insert the new sim and see to it that
the problem is gone now
