PUK stands for the PIN Unlock Key. You usually get this when you have entered the wrong PIN for 3 consecutive times.
You can get the PIN from the SIM card provider. It is usually on a small piece of paper that came along with the SIM card case. If you are on post-paid, ask assistance from your phone provider.
PUK code (PERSONAL UNLOCK KEY)is used to unlock the sim card if it is blocked.The mobile ask for PUK code if you have entered the PIN code wrongly 3 consecutive times.PUK code is provided by the network operators.If the PUK code is entered 10 times wrongly the sim card gets permenantly blocked.
If you get a PIN wrong too many times, you will be asked for a PUK. This is usually on your SIM package, or alternatively you can call your service provider for a PUK.

you will have to "block" your card by putting in the wrong password 3 times,you will then be asked for a puk code,when putting in your puk code,you will be asked to put in a new pin number. ask your service provider for your puk code.
call your phone service provider, you don't remember your puk code probably because it was never given to you, unless it came with the box of the sim card.
the puk is not related to the phone at all and the phone will work if you put in another sim card.
