PUK stands for Personal Unblocking Key. This is a unique eight-digit code specific to your SIM card. This is required when the PIN number to unlock your mobile phone is entered incorrectly 3 times. You need to contact the mobile provider and tell them your SIM is locked, and it is asking for the PUK CODE.

Your cell phone service provider can(Call from other no. n ask tell them for puk of ur no.) give you the PUK code aft3r answering simple security question.
Later enter PUK than enter new PIN, confirm new pin. Go to -Setting

In your case I believe your SIM is now FROZEN or locked. You mostlikely will need to purchase a new SIM card.
