Failed usb headers
1) Power down or reboot your system.
2) As the pc is restarting, press the F2 button to pull up the "Setup" screen.
3) Right Arrow to the "Advance" tab
4) Make sure that all devices are enabled
5) Arrow down to "Miscellaneous Devices"
6) Make sure "External USB Ports" is also enabled

hi mate, here is a few things to try btw i hope you used thermal paste when you replaced the GPU heatsink as if you boot with out the thermal paste there always a chance if you didnt that the gpu got hot fried in a few seconds ( i have seen it happen)
1) remove the graphics card and clean the motherboard socket it plugs into with a paint brush i noticed you did try a alternate slot but there still could be debris in the primary graphics slot causing a problem ( alot of the time dust from the cpu fan builds up on the graphics card slot and when you remove the card and put it back in the dust or debris gets pushed into the graphics socket on the motherboard )
2) while the graphics card is removed use a cotton tip dipped in isopropyl alcohol or metholated spirits if you cannot get isopropyl alcohol and clean the gold connectors on the graphics card where the card plugs into the motherboard
3) perform the same above 2 steps on the ram dimms and the ram slots incase there is debris or dust there
4) see if your pc has a onboard graphics port and try removing the graphics card and running the pc off the onboard graphics only if it works from the onboard graphics then you at least know its the graphics card and not the ram or board, if the onboard display doesn't work when the graphics card is removed then the problem is likely in the motherboard
5) test you motherboard encase its broken - remove you graphics card and the ram sticks and turn your pc on, it should beep loudly at you-
- if it beeps the board is generally fine
- if it doesnt beep when the ram and other cards are removed then it could be the motherboard
other then that your limited as to being able to test further unless have a second pc to test the card in or a second card to test in your main pc,

Ur fps depends upon the game ur playing. If ur playing high end game with a low graphics card ur fps will be low. Switch to a higher graphics card for good fps

then cross test YOUR card in other computer because it might be dead...
lucky that your motherboard is fine...
all those devices are so sensitive... sometimes they die you don't even know why...
