I have a koss ks4102
Source the TV-related audio from the Cable Box,. Treat it like any other audio device.
Audio
all KOSS unit i encountered are power amp ICs problem, might be yours. usually what comes first dead is the subwoofer channel. try, take that IC off first, and if it doesnt work, look for 10ohm resistor along the supply line going to the IC. to know where to find the supply pins, download the IC specs on the web such as
http://alldatasheet.com/. be careful when removing the IC, this is not an easy job as I handled it. The IC is under the heatsink. once you found the bad IC, the unit will on except that channel you have pulled the part off. its a surface mount IC, if your not use to solder components such as 0402 sizes, i would suggest to have it repaired on technician. hope this help.

Can't get remote to work
Since you don't know which end is defective, you might take the remote to about any store that sells and services consumer gear; they will have a primitive sensor that can detect if the remote is putting out some infrared but the common type of detector cannot tell you if the coding is correct or not.
If you find though that the remote is putting out anything at all, it is probably working properly.
Unfortunately that will mean that the receiving end is bad and that will be more costly to have repaired.

Problem with surround working with just the t.v. on
What type of plug from the TV are you hooking up to the sound system? Its has to be a HDMI or a scart. If its not and your using the rca red and white. The reciver has to be on the synth surrond setting or look in the owners manual, it will tell you what setting the reciver has to be on when using a non surround set or hookup
Video difficulty
You've probably given up on getting an answer since it has been some months but in case you haven't axed both yet . . .
You might try the DVD player on someone elses TV to isolate the problem to the player or the TV.
Or the reverse; one of the two is not synchronizing right and it is the TV's job to tweak itself to lock the picture from an external source.
It is unusual for any modern TV to have problems synchronizing with another device, even if they aren't too stable so I would suspect the DVD player first.

Mine does the same...i dont no how to fix it correctly but whenever i hold my tray in and dont let it pop back out it will read my disks...not really a solution but it works
