Take the domes off, wash them out or use a pipe cleaner to poke out the wax. If you have washed them, make sure they are thoroughly dry before you put them back on. When you put them back on make sure they are firmly on the receiver tip or you will be taking a trip to A&E to get it removed from your ear canal.

It could be a receiver issue, the cord, it might be a faulty receiver or alternatively it could be plugged with wax or badly affected by moisture. The instruments you show in the picture appear to be ear inserts rather than custom ear mold. Customs can also be small.

You may consider having a local Hearing Health Care Professional advise you as to the feasibility of replacing your sound tubes and inserts with Custom ear-molds.

I think you need to plug it into the tv instead since the sound is coming from the tv's speakers and not actually from the dvd player or? how are the things set up there.
