SOURCE: 1998 Jeep Grand Cherokee Power Steering Pump/Pulley/Belt

I see this is a two part question...
In replacing the alternator, there is no reason at all to be messing with the pulley on the power steering. That pulley is pressed on and though you could remove it with a puller it is far more difficult to re-install without the proper tools. (it is not hammered on). Power steering pulleys don't just fall off. They are so tightly pressed onto the shaft that they don't even require a retaining bolt to hold them on.
Without the benefit of being able to examine it myself, I'd have to guess that either you originally had a mis-match of pulley to pulley shaft and the pulley was sliding on the shaft (so remote a concept that you may be hit by a meteorite if you buy that) Or someone removed it for an unknown reason and did not properly re-install it.

SOURCE: putting belt back on pulleys

Your owners manual/label in engine compartment will have a diagram for how the belt runs. There will be one pulley that has a spring tensioner. This pulley keeps the belt tight and is used to install the new belt. Depending on the vehicle, these can be easy or very difficult to get at. The pulley or arm attached to the pulley will have a square hole of some means of inserting a tool to allow you to release the spring tension. I typically will install the tool and verify that I can get the tension off the belt before starting the process. Once I feel comfortable that I have the correct tool, I will release the tension and remove the belt. Then reinstall the new belt per the diagram. You will have to keep the tension off until the belt is in possition. I have found out that it is easiest if you put the belt on all the lower pulleys first and get down to one remaining pulley (top side of the engine so you have good access). Now use the tool inserted into the spring tensioner and move it until you have enough slack to get the belt over the last pulley, once in place release the tensioner. I typically have the belt over the tensioner pulley, and use the alternator or steering pulley as last pulley because it keeps your fingers out of the pinch points. There are some special tools you can purchase at Sears to assist, but I typically use a ratchet or wrench for this work. Sometimes a third hand helps out.

V-belt routing for a Chev 454 (7.5l) engine

This is the routing diagram for a 454 engine using v-belts only, not a serpentine belt.
The following abbreviations are used :
  • C - Crankshaft
  • S - Smog pump (A.I.R.) system
  • W/P - Water Pump
  • Alt - Alternator / Generator
  • A/C - Airconditioner
  • P/S - Power steering(In some installations also includes the brake booster pump)
This particular setup is most prevalent in motorhomes. Refer to the diagram.
Diagram 1 is for the first groove on the crank(closest to the block), and utilizes the 1st groove on the power steering.(belt no : 15415)
Diagram 2 is for the 2nd crank groove and utilises the 1st groove on the water pump pulley, the A/C pulley and the 2nd groove on the P/S pulley.(belt no 15590)
Diagram 3 is for the 3rd groove on the crank

If your motorhome uses a serpentine belt it replaces the belt in track number 3. (serpentine belt no : 6K560)
The upper smog pump(not shown in diagrams) is powered from the powersteering (Hydraboost) pulley (V-belt no : 15450)

What does a serpentine belt operate and do all the pulleys suppose to work 1995 winstar

I don't know that car in detail, but it appears that there is one belt, driven of course from the crankshaft, driving the A/C, alternator, power steering, and the water pump. It also has an idler pulley and a tensioner pulley. Quite a belting.

Need serpentine belt routing diagram for 1994 Dodge grand caravan

3.3L?
Looking at the engine from front (right side of the Van): Stating at the Top, Going Left to Right and Clockwise:
Over the Alternator
To under the Idle Pulley,
To Over the AC Compressor.
To the Left Side of the Water Pump.
To the Right side and Under the Crankshaft Pulley
To the Right side and Over and Down the Tensioner
To Under the Power Steering Pump
And Back up-to the Left side of the Alternator.
Advise when puting it on:
Put the Belt around everything else accept for the Alternator. After it is around all the rest of the Pulleys - Pull the belt up - tight from the Power steering Pump and the Idle Pulley to form a loop, holding the Belt close to the Altenator Pulley.
Using the 15mm wrench or belt tensioner bar - rotate the Tensioner Pulley towards the front of the Van and then place the belt over the Alternator.
Let me know if this helped or if you have additional information or questions. Feel free to contact me at FixYa.com!

Starting at the crankshaft pulley, and going clockwise, the belt leaves the crank pulley,
Goes around the tensioner,
Goes around the water pump
Goes around the alternator
Goes around the upper idler
Goes around the Power Steering pump
Goes around the air conditioner pulley
then goes back to the Crank pulley
