Make sure that the belt is fully seated in ALL pulleys, if it still slips off, then you have a pully that is bent, or out of alignment.

It is much easier, and cheaper to find a steering knuckle or "hub assembly" in a salvage yard.
To begin, jack up the car and remove the wheel. Be certain the car is stable and will not roll off jack(s)!
Remove the spindle nut. Remove the brake caliper (hang on a wire). Remove the bolt for lower ball join. Remove the bolt that goes thru strut. Now, pull assembly off while prying to ease pressure from the lower control arm.
Depending on how many miles you have on the car, you might want to head-off some future trouble by replacing the half-axle while you have the front end disassembled.

Everything worked as mentionned. Be carefull with the plastic in the winter. Make sure to use a flat screw driver and get it close to the slider and not on the edge. I cracked one of the tongues. Those L shaped slots are under the plastic cover with the two big plastic plugs that you also remove with the flat screw driver

First - note the routing decal on the underside of the hood. If there is no decal, draw a diagram before removing the old one. Move the tensioner using a breaker bar (if there is a hole for it) or a wrench. Slide the old belt off the pulleys and put the new one on in the same position.

Its possible that your belt may not be correctly routed or tensioned.

Check your 3.1L engine accessory drive belt routing. The arrow points in the direction in which the drive belt moves.

NOTE
Routine inspection of the belt may reveal cracks in the belt ribs. These cracks will not impair the belt performance. The belt should be replaced if the belt slip occurs if a section of the belt ribs are missing. A single serpentine drive belt is used to drive all engine accessories. All belt driven accessories are rigidly mounted with the belt tension maintained by a spring loaded tensioner. The indicator on the movable portion of the tensioner must be within the limits of the marks of the stationary portion of the tensioner. Any reading outside of these limits indicates either a defective belt or tensioner.

(1) Remove engine mount.
(2) Rotate the drive belt tensioner in a clockwise motion using a breaker bar or serpentine belt tool.
(3) Remove the drive belt.

To install:

(1) Rotate the drive belt tensioner in a clockwise motion using a breaker bar or serpentine belt tool.
(2) Install the drive belt. Connect the serpentine drive belt last to the power steering pulley.
(3) Install engine mount.

Need step-by-step instructions for installing a serpentine belt on a 6 cyl. 1997 Chevy Malibu.

The path should be listed under the hood on a decal, you will need a breakerbar and apropriate sized socket to compress the idler to put the belt over that pulley... just remember to put the breaker bar through the belt before compressing the idler assembly .

99 Chevy Malibu belt chirping noise problem.

one or more of the pulley's is defective, most likely the tensioner pulley, but check them all for wear and side to side movement which indicates a worn bearing, this causes the belt to not run true and chirp.

Serpentine belt slips off idler pulley

The A/C pully is supposed to spin freely until the AC is activated to run.. The cranshaft pully does not. Replace the tensioner pully. The belt has to fit in between the grooves on the pullys.

if one of the pulleys(power steering is not uncommon) have been removed it is possible it was not pressed back on to the pump shaft far enough. this will cause the wear you explained

belt tensioner is not doing its job!! but with out looking at it is hard to tell. i f the crank pulley is not bent or harmed, it is probably the tensioner.
