I will state the obvious has the timing belt been checked because if the cam shaft is not turning then all the problems listed will be happening. Either that or check the condition of the key fob battery as the immobilizer will cause the same problems.
Defroster not working
your defroster may be interrupted and needs to be replaced. It can have damages that at tines are hard to be seen. it is connected to a small plug it may be disconnected or check all your fuses.

mass airflow sensors don't have a vacuum line, you sure you don't mean the EGR valve? if it was the regulator i doubt it would run good or make much of a difference unplugging the vacuum line, take a reading of the fuel pressure, if it starts to drop while the van is running you have a bad regulator.

It would help to know specifically what van.
But in general, there would also need to be some kind of temp sensors. If what ever van you have just uses the ECM to turn the fans on and off then it may be a bad computer temp sensor.
The ECM also needs to send a ground signal to the relay when it gets the right temp readings. You can also check for power at the relays.
There could also be a temp sender elsewhere like in the radiator or cylinder head that will supply a ground when the heat is up.
You can get free diagrams from AutoZone.com in the repair section. These should show you everything in the circuit and give an idea of what to check.
