TPS SENSOR. IT CONTROLS SHIFTING AT THE RIGHT TIME AND COULD ALSO CAUSE ALL OF THE SYNMTHOMS THAT YOU HAVE LISTED SOMETIMES WITH OUT GIVING YOU A CODE GOOD LUCK
Assuming you have an automatic transmission....Sounds like you probably have some internal damage. Take it to a reputable transmission shop and have it diagnosed.

Before guessing at transmission problems, check the fluid level (car warmed up, engine running in Park on level ground) and have the PCM scanned for trouble codes FIRST. Shift suddenly become very hard. When going from park to either reverse or drive it slams into gear. 1-2 and 2-3 shifts are also harsh. All gears seems to work. Problem may be intermittent: Check TPS for smooth electrical response. If the response is jumpy or erratic at all, replace TPS. Thank you for using fixya and keep us posted.
I have a 2002 Hyundai Sonata and I had the same problem. But mine would always shift into 3rd gear and STAY there until I turned the car off. I took it to a Hyundai dealer and he had to rewire a speed sensor on top of the transmission. Apparently a little animal got up in that tight little spot and gnawed on my sensor wires. The car has run fine since. To make a long story short, it might be your speed sensor on your transmission.
P1723 A/t control relay - Stuck on.
This Code is showing that the main control relay of the transmission is
stuck on. When that happens transmission goes in to the fail safe mode
which is being stuck in 3rd gear to prevent damage to the transmission
