Your clutch master cylinder may need to be replaced. This is where I would start as its cheaper than doing a clutch replacment. I would take the car to a tranny shop and have them do the free estimate. Make them tell you whats wrong and go from there...
The clutch and pressure plate is just worn out you will need to remove the transmission assembly to replace the clutch and pressure plate

hi sounds like the friction plate discs springs are weak not the cover plate if you do fast gearchanges the plates springs will break completely change the gears normally and it will last yates210456

Unfortunately you have a hydraulic clutch, which is self adjusting on your car.
The problem you are experiencing is the excessive wear to your clutch, or oil contamination on the clutch plate.
Both these problems can only be fully diagnosed once the gearbox is removed and the clutch is inspected.
There is also a possibility of excessive wear, or scoring, on the flywheel surface, the longer you drive the vehicle with a slipping clutch, possibly requiring the flywheel to be skimmed.
