You might be floating a valve. If it has valves and springs I would test the intake valve spring for spec, if it has reed valves, I would just change out the reed.
I would almost garuntee you have a bad water pump. When water pumps get old there turbines can wear off and even though they may still be there they want be as efficient. When this occurs you can normally rev the vehicles r.p.m. to about 2000 rpm and the heat will start working.
