Without a steady idle rate, its bound to act up when there s load on it cos with the fluctuation the idle might be too low at some point. Things to look at will be the idle control solenoid, throttle body, MAF sensor, check if carbon tracked then clean out throttle body with carbeurator cleaner.

Something mechanical, ignition, or fuel.
It can run rough under a load such as in gear and ok in neutral with no load. Have you checked the plugs and dist cap ?

I have a similar problem. The engine run fine at idle but hesitate if increase the rpm with no load. If I pump it hard it is ok. I am not able to find the problem yet.

What happens when you unplug and cap the vacuum line at the EGR valve? Does the problem go away? Before doing this does the car idle rough? If so perhaps the EGR is stuck open...
