now , because it showing two types of flash code, sounds like a Curtis 1212 controller now the flash code means

sound serious, well it can be a controller fault ,, it simply means the circuit that drives the relay has become faulty .

so what does that mean well ?
most likely the connection between the read axle and the frame so check them out , making sure they are a good connection, maybe use some contract spray, if the axle is not removed often then I suggest removing the axle and try refitting it may well sort the issue .
