You either have the DVD player plugged into the wrong input on the TV or the TV not set to that input. If using HDMI change the HDMI cord to another HDMI port on the the TV. In an Extreme case you have a bad connecting cord (either HDMI or other type.)
DVD's (CD red book standard) are 120mm (12cm) in diameter the smaller 8cm disc's are the 1.4gb versions. not all "slot" loading mechanisms can accept these smaller type of discs. if you are trying to insert 120mm discs then there is a fault with the mech!
Change input device to some other like a DVD player and see. Switch off TV, switch off power, remove plug and wait for 48 hours and retry. If problem persists, call in a qualified service technician.

This may be an instance were the idea was great but the execution wasn't thought out. I've heard of several related problems that may be related. Both the TV and the DVD player tend to produce heat, not in great abundance, but together a sizable amount of heat. The power supply may have been ample for the TV at first but with the addition of the DVD player was not able to handle the extra load. If still under warranty you're all set. If not you might call Akai and see what they might be able to do for you.
The AKAI LCT3201AD I got from a friend who didn't know it was stuck in stand by. Took it to the local TV repair guy who charged me $25 deposit to check it out and replaced the power supply for $99 total. Works good except for DVD player that won't eject. Just need to figure out what bolts are to be used for mounting to the wall.
there are capacitors on the power supply that will be vented or the top bulged out
those will need to be repalced to correct the problem
