L5 sometimes may refer to LS(Low Speed). There might be a problem with your motor sensor. It is best to call the place where you purchased this device for possible replacement if a reboot will not suffice.
The receptacle is for 120 volt 3 wire connection. The white and black wires would be connectyed to the screw terminals on the receptacle. the red would not be used in this case, just the black, white , and green.
I had this happen on our machine after about 5 years of use. I just recalibrated it and it appears to be working fine now. See the manual for information on the steps needed to take in order to do this.
