If you have forgotten your pass code, and if you have to remove it to sign into Xbox LIVE, follow these steps:
Note The following steps apply only to Xbox LIVE online profiles. When you delete an offline profile, you lose any Gamerscore that is associated with the profile. You also lose achievements and statistics that are associated with the profile.
Delete the profile for which you forgot the pass code. To delete a profile, follow these steps:
Go to the Settings hub and select System.
Select Storage.
Select the memory device on which your profile is stored.
Select Gamer Profiles.
Select the profile that you want to delete.
Select Delete. When you are prompted, select Delete Profile Only.
Press the Guide button and select Download Profile.
Follow on-screen instructions to download your Xbox LIVE profile.
If you want to set the pass code again, follow the steps to set an Xbox LIVE pass code.
Go to My Xbox>System Settings>Memory on your xbox dashboard. Delete your profile only (not profile and games) from the hard drive and then recover it to the console. To recover your gamertag to your console, all you need is the email address and password associated with the account.
You'll need to use recovery game tag, and verify your status with MSN Microsoft, Think about changing your password, could your profile is being used at a friends house, maybe add a pass code to your profile.

for both the console and for Xbox LIVE accounts, you can set pass codes
for both. Here you can learn how to:
Set an Xbox LIVE Pass Code When
you set an Xbox LIVE pass code for your profile, anyone who wants to
use that profile will need to enter the pass code before signing in to
Xbox LIVE. To set an Xbox LIVE pass code:
Go to My Xbox and open your profile.
Select Manage Account, Xbox LIVE Pass Code.
Enter a four-button code using the X andY buttons, triggers, bumpers, and directional pad on your controller.
Confirm your code by entering it again.
To reset your pass code, repeat this procedure at any time. (You

Set a Family Settings Pass Code When

Its not a big problem as long as you have a recovery question set up with the account.
Try going to http://msn.com and trying to recover a password with the email you provided.
If not, you just have to try and remember it.

You may very well have a bug in one of your saved files/downloads. To fix this issue, what you will need to do, is delete your gamertag, basically to refresh your xbox HDD, then go to xbox live and go to RECOVER GAAMERTAG,place the email address you used to originally set it up, and recover it. this HAS happened to me once before, and this has a 99.99% of working, now before you start worring, you will not lose your gamertag altogether,....unless you forgot your email address.
