You will have to be more specific. 12v adaptor for what? What brand Monitor is it? Does it have a model number? Usually the model is on a large sticker on the back of the monitor near or behind the stand bracket.

If the buttons are micro-switch type buttons (like you would have on a car's alarm remote) and they don't have the proper feel, then the monitor is probably ready for the recycle bin. Doing a little looking around it appears that the monitor you have is prone to failure. I think you should look into a replacement. Samsung makes a great monitor these days. someone set the refresh rate or the resolution to high for the monitor to display - start up in sage mode and change the resolution to 1024 X 768 this should work

5 seconds? Hmm, too long for inverter driver turning into protection caused by bad lamps. Also too long for PWM controller in the switching power supply.
Rather looks like overheating of the FETs in the inverter, they are driving HV-transformers. Open the monitor, remove metal lid and observe inverter board. If PCB under and around these FETs is darkened, replace them.
These Dells hard to open. Take the foot off and unscrew plastic cover. Then using flat screwdriver with wide blade separate plastic halves of the case, starting from bottom edge and moving to opposite. It might be sweaty.

The "image" you are talking about: Is it there when the monitor is off? If so, you are correct--it's broken and must be replaced.
Is it on the desktop when you start the computer and does it stay there no matter what program you are running. If the answer is "yes" then I'm afraid the answer is the same.

Have you tried the monitor on another computer? If so, and it works, the issue is with your graphics settings on your computer. LCD and plasma based monitors often come with a protection circuit that disables the monitor if the synch or refresh rate are beyond it's capabilities. this is to protect your monitor from damage.

Do not exceed your monitor's max resolution.
Resolution is determined the following way:

If it doesn't work then you can recycle it. If isn't working, do not pay someone to look at it. You will end up paying more than you paid for it. Get a new one

Not sure that you are talking about a blood pressure monitor or a heart rate monitor.

if you are talking about a heart rate monitor , which it sounds like because those have watches and chest bands that is something different. i would need to know what kind of heart rate monitor it is. A few tips on the devices, you will need to dampen the contacts on the band, make sure you place it in direct contact with your skin, under your pectoral muscle, if you are female then it goes under your sports bra band. i always reccomend jumping on a machine for a little warm up then starting the watch.

I noticed you said that you changed the battery i hope you sent the watch in to the manufacturer to change, sometimes it comes out ok but alot of times it doesn't work there are sensative devices in the watch that can get messed up very easily.
