It's below freezing (32 degrees F) when it has the most trouble. The hesitating & stalling only happens if I sit at a light too long right after starting it. I've tried holding the gas down while sitting at a light to keep the RPM up while it idles--not sure if it helps, but I haven't stalled since implementing that technique.
You don't say what make of car but it's about the same for all. Your problem is the choke adjustment. The choke raises the idle when the car is cold and after warm up drops to normal. You are doing the chokes job when you hold the pedal down, once its warmed up the choke no longer functions till cold again. So check the choke and the high idle speed adjustment. Simple fix. The 5th injector acts like a choke on a carburetor. It probably isn't working or the sensor that operates it isn't functioning.

You have one of two very common problems.
Number one may be a stuck open EGR valve
Number two would be a failing Idle Air Control valve (IAC).
Try this: Open the hood & locate the EGR valve, close to the firewall, behind the intake. Find & take with you a small hammer. The next time that it happens, open the hood & tap on the EGR valve. If it is sticking "open" the tapping may dislodge the carbon particles that are keeping it open.When the EGR valve is open, exhaust is getting into the intake at idle, and this will def stall out the car.
There is no repair to the IAC valve, it would need to be replaced if it is bad.
One of these two scenarios will cure your problem, I see it every day.

Don't have a good answer to your problem but the items I'd change would be the coolant temp sensor and the manifold air temp sensor if yours has one. Both will influence the condition you have posted. Also, the rpm's you noted are on the high side of safe on normal cold starts but because in extreme cold the engine oil takes a while to get where it needs to be, I'd consider using a good synthetic. It flows easier and stays on parts without draining off, giving a bit more protection.

It sounds like you may have picked up some small debris from the bottom of the gas tank when you ran it empty. You may need to run a fuel/injector cleaning solution through the next tank of gas to clear the debris and consider changing your fuel filter too. Sea Foam is a great fuel additive and I recomment trying it full strength. Hope this helps!
