Run the fault codes to find the faulty sensors. It may be faulty injectors or injector control that is fixed when there is a need for more fuel ( in gear)

Check your motor mounts, recently engine idled rough when in nuetral or stopped in drive, also jerked when put in reverse. took it to a automotive shop and found motor mounts cracked. Runs great now

BAD SPARK PLUGS AND WIRES DIRTY FUEL INJECTORS AND STOPPED UP FUEL FILTER OR WEAK FUEL PUMP WILL CAUSE ENGINE TO SURGE.

1997 Mitsubishi Galant When idling or neutral runs fine, when I put it in gear it shakes something awful until it gets up to speed, almost stalls. It is very hard to get moving shaking and rattling.

Check your engine mounts, shaking and rattling could be vibration running in through the car= rubber perished on mounts.

2005 chrysler town and country when i start the van it shaking vibration hard in park and in reverse it and in neutral and in drive it perfect.it drives good on highways no shaking

try to increase the idle-rpm..
Example - at standstil condition when you start your van, have look at rpm... say 1000 or 1200 or 1500.. whatever it may be..
Keep idle-rpm a little bit higher, if previously it was 1200rpm then set it to 1500 rpm..
If the problem is solved then..
1) clean ur airfilter
2) check spark plug condition
good or bad... if bad then replace..
3) check fuel supply line, if it is damaged or folded

If it were an IAC problem it would do it in reverse also. A bad engine mount can cause shaking in one direction but not the other, Also check your vacum hoses and lines.
