Backup your phone then do a Remove Memory card and Press *#7370# and then your passcode(Default 12345) it will Format and Reset all the settings to Default Factory settings. Then Check whether it happens or not.

Hey there, you should contact the customer service of your telephone provider. You have to tell them about your phone number, and the numbers located at the back of your sim card. I think it's a 16-digit code. They will tell you the PUK code

If you format the phone the can only be recover from your sim card. If not formatting will erase all the information on your phone.
