PUK codes can be obtained through the following ways -
The PUK code is in your SIM card documentation Starter Pack.If you do not have this then -Please contact your phone service provider.
Your cell phone service provider can give you the PUK code once you give themyour account details and they have confirmed your identity.
For getting PUK code of a Airtel number, send PUKPhoneNumber to toll free number 121. You can make use of any other Airtel number to send this message since your SIM might have been blocked already. If you are not on Airtel , call customer care from a friends phone (same carrier) ...they may ask you a few questions for verification

dont try more than 3 times.If you try more than 3 times the Sim will get
locked.And it will ask for PIN code.Both these PUK and PIN code can be given
only by the network service provider. Please call the network service provider
of your mobile.And ask him to provide the PUK code or PIN code.He will ask you
some basic details like name,date of birth for his verification.And he
will provide you the PUK and PIN
code.This is done for free of cost.
