This is common of advertisers to do this. Some higher end TVs can auto adjust the volume to keep it at a more even level but commercials are usually louder on purpose. The FCC has just passed a bill to limit this practice. You can read more about it here:

Go to your menu, system setup, then there should be an audio menu. Change it to L-PCM. (How to get there may vary from receiver to receiver, and on cable boxes may or may not have setting)

OR

your receaver (or what your tv is connected to) Is causeing the buzz.

Why doesnt the volume on my remote control work?

The wrong code on the remote has been used. Though some codes will work parts of the system only the right code will operate all.
If you don't know the code for the TV, then you will have to try ALL the codes until you find one that operates all the controls.

I have a Vizio 47 inch LCD HDTV hooked to a Dish system. The volume fluctuates from very loud to very soft between ads and programs. It also goes from moderate volume to very soft within a program. Help!

Either unplug dish from tv or use the antenna side which is HD and program channels and see if volume the same, if not and is OK then it's your Dish bring your sound down or audio cables bad.

Too loud

I am assuming you do not have a "surround sound" system hooked up to your digital cable box, and that you have your TV volume turned down all the way and have not yet tried to adjust the volume on your new digital cable box. It might be worth a try to unplug your TV for about 15 seconds and try again. If the above is true and you can still not lower the volume through the cable box or the TV set, then I would have the cable people stop by to check it out.
