Turn the computer off.
Disconnect the AC power.
Then, since you need to ask "how" to do it, I strongly recommend that you take the computer to a qualified technician, for _professional_ repair.
What you're wanting to do is _very_ complicated, and you could really "break" things, as you try to disassemble your computer, without breaking any wires that connect the motherboard to the LCD screen.

First off have you tried doing a system restore? That is the first thing I would try. If your unable to restore. I would back up any valuable data and reinstall windows with either a copy of windows or if there is a recovery partition on the hard drive. If you need further assistance please let me know.

Head over to Toshiba's support site for your laptop here. Switch All Categories to Camera and download the web camera application. This contains the web cam driver.

If the USB ports are loose, then the computer is going to have to be disassembled for repair.You should have Toshiba, or a Toshiba authorized service center fix it. If I remember correctly,all you have to do with Toshiba laptops is just drop them off at UPS for repair.UPS sends them to Toshiba for free, and Toshiba returns the repaired laptop to you.
