Hi richardsnead dial *22890 and this will reset the lock code to the last four digitals of your phone number. Also you can take it to the nearest corporate verizon store so they can get your lock code for the phone. Hope this helps

u cant reset the lock code by your own, its default code is 0000. but if u changed and forgot then u must go to the near by service centre they will charge u very less for it
