Open the left cover and check for obstructions in the paper path (paper,foreign substances).
Check the paper tray bottom plate (the metal one) on which the paper stack is sitting and ensure that there is at least a 1mm gap between the metal edge and the plastic wall to the left.
Do you have the same problem when feeding from all trays?
If that happens from all trays then a technitian needs to check the machine. The best case scenario is that the registration sensor is blocked or damaged. The worst case scenario is that the MCU board needs to be replaced.
The document feed rollers may be dirty/worn which is causing the paper to come into the scan area late causing your document cut offs. use a clean, lint-free cloth dampened (not moist) with water and clean the document feed rollers. Confirm they are dry then, try scanning multiple pages and see if they pull through without stalling. If they do, it's time to replace those rollers.

This is a fuser error, so we'd start by replacing that. The fuser PN is 126K16480.

Yep, easy fix. Look into the area where your copies come out, between the paper try and scanner. look towards the upper back and you will see a flag sensor, about 2 inches wide and 1 inch in height. It has come out of the 2 clips it snaps into. Carefuly move it around until you line it up properly then snap it back into the clips it mounts to. You may have to gently squeeze those clips together to hold the flag in place.
That is your problem. Easy one to resolve.
