Sounds like it somehow got overloaded. Did anyone step on them? Single strollers are typically tested to hold up to 100 pounds, which is well above the weight of a child and their gear. Regardless of what happened, contact Evenflo directy and see if they will replace the rear axle and wheels. You can't by replacment wheels at any store.
Inspect the stroller's wheels and handle to identify the type of brake mechanism it has. If the device is equipped with wheel-mounted brake levers and swivel locks, use the toe of your shoe to activate these mechanisms. If your stroller is equipped with a handle-mounted brake lever, push down on the lever to engage the brakes. Some models also come equipped with a secondary-locking mechanism located on one of the rear crossbars; use the toe of your shoe to activate this feature. Lift the handle or press the tab securing the backrest in place, located on the upper edge of the backrest or on the crossbars securing the rear of the backrest to the frame. Grip the edges of the footrest, then lower the component by pushing it into the desired position. Pull back the stroller's canopy. Grip the stroller's handle and lift the device upward; if the stroller comes equipped with a safety button, press the button as you lift. Use the toe of your shoe to push on the stroller's hinges until the frame begins to fold. Pull the halves of the stroller's frame together until you have completely folded the device; if the stroller comes equipped with a locking latch, engage the latch to keep the halves from sliding apart.
