I have a similar problem with my late 1990's Wurlitzer OMT Juke Box. The Cd goes over to the laser and tries to play 4 times (every time/everyCD) and returns to the carousel. I have looked at the fuses but they seem OK?
Also the display defaults to 2222 when the unit is turned on. The correct CD is always selected and the number pad works. The display also (always) reads 47 when I select the R button. Do I have a dud of a Juke Box?

Fault could be fuse in cd transformer or decoder board worst it could be the laser unit
When the disc is taken from the carousel and put on the player how long does it take for it from the time it is placed onto the player to being returned to the carousel? Also is the cd spinning when its on the player or not spinning at all. The answers to these questions should help in pinpointing the actual problem.

Check to be sure the cd in question plays in another CD player. Also I by mistake put a DVD disc into the unit from a collection and since it could not read the DVD that was the result is that it could not see information so it tries to spin it backwards. That was a true mistake. Eventually it will lock out that selection. If it is not reading cds, be sure to power down and reseat all of the cables coming into the controller board that sits behind the mechanism mounted at the back of the box. I found that the cables over time develop possible corrosion of the gold pins.So just taking them on and off several times cures problems. There is also an Initialization, but on Wurlitzers you can still make the selections one at a time for each slot, and it will accomplish same thing. Highly reccomend purchasing a manual that has tons of good information and shows how to program it and many other options.
