Regarding to problem you encounter first, You need to call the phone company that sold you the phone card to unlock the phone.The PUK code is asked if you enter a wrong PIN 3 times. Second, You need to call the phone company that released the SIM card, they can retrieve the puk code from a database for free. Third, In your case you need to contact your SIM card phone provider technical support to unlock the SIM.
Note: The PIN and PUK are related to the SIM card, not to the phone. If you replace the SIM card with a different one, then the phone will not ask for the PUK anymore.
PUK code (PUK stands for Pin Unlocking C(K)ode)is asked when you enter incorrect pin consequently for 3 times. There is no way in which you can guess your PUK code. What you have to do is, call your customer care from another mobile of the same SIM provider and report them the same. Then they will ask you some questions as a security measure and will straight away give you the 10 digit PUK code. It's so simple.

loose any money due to call charges.
There are two ways to get your PUK number. The
first way you can get your PUK is to consult your documentation that
came with your SIM Card. Somewhere your PUK will be given within the
documentation.
The other way to get your PUK if you don't have
your documentation or lost it, is to phone your mobile service provider.
They will ask you a few details and then be able to give you the PUK
code for the SIM card.

If you need the PUK ( Personal Unblocking Key ) for your phone, you will have to contact your mobile service provider.
PUK code is related to the SIM card you are using. So, kindly call the Customer support of your mobile service provider and they will provide you with the PUK code.
Also, entering a wrong PUK code ten times in a row disables the SIM card permanently. So, please be careful while entering the PUK code.
