Sounds like the bolts that hold the motor assembly on the table may be loose allowing the blade assembly to move. Look underneath the table and clean out all the dirt and loosen all the hold down bolts and realign the blade and tighten back up.
Use a carpenter square, place the square on the table top with the blade up right, adjust the tilt of the table saw blade until it is exactly parallel to the blade of the carpenter square. Check the blade in several places as the table saw blade is sometimes warped. Try to adjust until a happy median is obtained.
Possible that the fence you are using is not on a true parallel line with the blade and as a result it is causing the wood to pull into the blade causing kickback.
I would do an alignment procedure on the fence to make it as true to the blade as possible.
Remove the drive belt, loosen the motor base bolts on the motor and put a straight edge on the outside edge motor pully rotate the motor till the straight edge lines up with the outside edge of the saw blade pully then retighten the motor base bolts and replace the drive belt.
The guide is called a fence. The fence should be positioned to the right of the blade. When using the fence, the largest part of the board goes between the blade and fence. Fences are typically have some relief toward the end, so you don't get the blade cutting on it's way down, and then cutting again on it's way up.
Sounds like your table is tilted. Put a small square on the table against the blade, then adj the table to it. If that is not it, then the blade guides are off up and down, back off all the guides, wheels, turn the blade by hand, get it tracking in the center of the tire, then bring the bearings up the blade, don't push it , just touch it, then bring in the guides, again, make sure it doesn't push it, the guides stop it from going off, it should track by itself. The blade lines upaccording to the rotor shaft and has nothing with the guide except for the fact that the table has been droped and bent out of place or has been tweeked out of place somehow, usually hitting the floor from the table top causes this. Loosen the table screw to see if you can get it back in place and square it back up, hope this helps, good luck!
