I had this problem with my Orange Hero too - there is a fix but it's quite obscure.
When you're inside the "choose an alarm sound" menu and scrolling through the list,
if you press the volume buttons on the side of the phone then you can manually adjust
the alarm sound (mine was set to 0). Not sure what causes it to set to zero,
presumably when you take the phone off silent mode it sometimes fails to update the
alarm as well? This still needs looking at because it means the alarm volume resets
to zero of its own accord, but it can at least be managed by manual readjustment.

P.S. It might also be worth noting that when I was having this problem, no
third-party alarm apps would play the stock alarm sounds either.
Alarm volume is set to silent by default. when setting your alarm, go into the list where you can pick your alarm sound, and use the volume control to set it louder. there is no other place on the phone to set the alarm volume.
When you have set the alarm, select "Alarm sound". Pick another sound than already selected and change the volume on the volume-buttons on the left of the phone. This worked with mine!
I could only fix this by uninstalling all of the extra applications. Then removing the battery for 30 minutes. I was also having problems not receiving text message. This appears to fix both problems. I will reinstall certain applications to see which one created a problem. However was probably the use of one application after a 2nd application that caused it

I don't want to question your common sense, but have you changed the AM/PM setting for your alarms? I am trying to think of anything that could be the cause, but your alarms work when it set it for a few minutes later, this is the only setting I can think of.
