How do I clean a lens of a phone camera - the lens itself, not the protective glass?

This is quite common, if its what I think it is. The lens needs buffing down past the protective film, which is what's causing the 'fog' effect. For some reason it goes slightly opaque on some phones.

You'll need a soft cloth or lens cleaning kit of some kind, or glass cleaner... anything that will clean glass really well.

Take the back off the phone, spray some cleaner on a cloth/rag/glasses cleaner and start to polish the lens. You may/should see the tint coming off. It takes some time. I took me about 10 minutes to do my xperia mini, which had the same issue. Stick with it and to see if what you are doing is working take a few pictures and look at the results.

Although you can make pictures with a phone, don't expect the same quality you would get with a real camera. The quality can be influenced in a negative way, because most phones are in a pocket, without any protection for the lens. So the lans can be scratched or smeared with grease.
Check if the lens is clean. Never try to clean a lens with fluids like alcohol. Only use a soft cloth, you also would use to clean your glasses, or a small type of lens pen. Check if there are any scratches on the lens, with a magnifier glass. To improve pictures taken with a phone, make sure there is enough light. Poor light conditions can be affecting pictures made by a normal camera, so a phone will be even more affected.

More like loose focus, need to open camera, focus one more time

It seems that hairspray has hardened on your lens.I would at least try some isopropyl alcohol(rubbing alcohol).It won't hurt the glass.If that doesn't work goo-gone should and should not hurt the glass but I am not sure about the plastic.I think it will be okay,just read the precautions.It sounds like if you don't try these at least,you're going to need a new lens anyway?

when the len's comes out, try cleaning the glass part of the shutter with your shirt or a clean cloth. This worked for me.

you just need to replace the housing of the phone to correct this issue.

Remove back cover. Once removed you will see four screws they need to be removed using a T6 screwdriver. After removing the screws carefully remove the panel. Turn the panel over and clean the inside of the lens cover. Use either a soft bristled brush or a lens cloth to clean. After cleaning reassemble and test it out.

yes you can have the other lens replaced just take the phone to a mobile repair center.
