The PUK code is a unique code provided if incase your sim card was blocked normally it came with the sim card that was provided by your service provider.
If you cant find the PUK code. you must need to contact your service provider and tell them that you need your PUK because your sim was blocked.

There is one way to access your phone and that is: Block your sim by entering wrong lock code and then it will ask for the PUK code that you will get from your service provider. Enter that PUK code and your phone is unlocked. By this method you will not loose your data in your phone. Thanks..
