It seems that the card is bad. Try to format the card on a different machine to rule out your Windows formatter.

I experienced the same problem (my USB device ). try these steps:
1. Start, Control Panel,
2. Performance and Maintenance,
3. Administrative Tools,
4. Computer Management,
5. Disk Management,
6. Find the USB drive (usually the last storage device before the optical drive),
7. Click on the USB drive,
8. Right click on it and you will see the options (New Partition or Format)
9. Follow the instruction (I chose FAT32), but please do NOT chose Quick Format.

External hard drive cant format into ntfs but only exfat

The propriety NTFS file format is used by the Windows operating system to format internal as well as external hard drives. You can format an external hard drive with the NTFS format using the built-in program included with the Windows operating system. The procedure is straightforward and does not require any additional software. An NTFS-formatted external hard drive can then be used with any PC running a version of Windows that is later than Windows 3.

Plug the cable attached to the external hard drive into a USB port on the PC. Right-click on the "My Computer" icon on the desktop. Select "Manage" from the drop-down menu. Click on "Device Manager" in the left column of the window that appears. Click on the "+" icon next to "Disk drives" that is in the column to the right of "Device Manager." Right-click on the icon of the external hard drive. Select "Properties" from the pop-up window. Click the "Polices" tab at the top of the window that has replaced the pop-up window. Click the "Optimize for performance" button below "Policies." Click "OK" to close the window. Right-click on the icon of the external hard drive in the center column of the window that is still open. Select "Format" from the pop-up menu. Select "NTFS from the "File system" drop-down menu in the new window that appears. Click the "Start" button at the bottom of the window. Wait as a progress bar appears at the bottom of the window and fills in from left to right. Click the red "X" at the right corner of the window once the progress bar has completely filled in. format usb memory stick to ntfs http://www.online-tech-tips.com/computer-tips/format-usb-ntfs/

Hello-:~) i just wanna ask why is that my transcend is unable to format whe i use to click the format windows unable to complete format will appear?? how can i format my USB

You are plugging it in, going to windows, right clicking the drive - format - quick format and its not working?

Try right clicking the drive, going to properties, and run the error checking or disk checking for errors. Windows can attempt to clear the problem.

Boots to progress bar before loading windows

It seems that windows tries to check your Hard Disk for errors before loading the system files to memory. Do you hear some sounds from your hard disk? If you can successfully get to windows, open my computer => right click on drive C => Click on Properties => Click on TOOLS Tab and click CHECK NOW button.

Check the next 2 options and answer YES when windows ask you to schedule a thorough scan during next boot.

Regards!

Unable to format the cruser pendrive

Far too often these little things have an almost invisible read/write protect button on them. Look carefully, over the entire surface of the USB Flash Drive. You might have this switch turned to Write Protect, which means there is nothing you can do, until you turn it the other way, and allow the device to be written to.

Cheers!
