SOURCE: bike pedal arm keeps falling off as i pedal.
You need a new crank arm. If you ride with the nut loosened what happens is the square part of the bottom bracket shaft wears the hole out in the aluminum crank and it no longer fits snugly no matter how much you tighten it down.

I looked into this thinking is must be a simple belt, or tention wheel, but these devices are much more complex than the average exercise machine. See the Recumbent selection here.
It appears that the tentioning device is a magnetic brake.
This would require that it be plugged into a power source and programmed to provide a desired tention.
I've never used one of these so I can't say what kind of programming options there are or where they are found.

Yes, the crank-arm could be replaced.
1.You would need to remove the flat nut holding the crank to the frame. And then take out the bearing.
2. A special open-ended wrench is required to remove this nut. Please visit your local bicycle shop for a new crank-arm, and the required tools, also.
3. Put in new crank-arm and its bearing.
4. Grease the bearing.
5. Tighten nut.
6. Give it a test ride.

This unit takes a special 6 volt battery. The battery is in one of two places. First place to look is under the unit. If you lay the bike over on its side there is an opening in the covers. If the battery is not there then you will need to take one of the side covers off. When you get the side off you cannot miss it. The battery is black with three cylinders.
