Can you give us a little more information? cause blocking a sim can be disabled on dual sim cell phones. but if your phone came from the same network or with plan, i don't think you may be able to that, try to talk to your network provider on this matter.

You need to contact your mobile phone service provider, they are the people who can unlock the SIM card.

Do not enter any code into your phone. The PUK code is a code that your service provider has and not one that you would know or have in your manual. If your an AT&T customer they do provide PUK on their website. If you have entered in a code and the phone is now saying PUK blocked you'll have to go to store to purchase a new SIM card.
