Need help with activating sim card tried last nite didn't work

Your handset can become fully locked due to exceeding the number of attempts to enter the correct unlock code. Regarding your Sim from Optus I believe that you will need to call Optus to obtain the PUK code, unless you still have the card where the Sim card was removed from. it will have a PUK at the back which you will just need to rub off to get your PUK.
You can try 1234, but if this does not work you will really need to call Optus since they would know better how to get the PUK for your Sim if we try entering a PUK code several times that we are not sure of, we may disable or damage the Sim card more. Call Optus for further details regarding the PUK.

there might be problem with the sim card holder. try to insert some piece of paper folded atop the sim card and close the sim holder

call customer support, they might have activated your call barring feature.
