only the service provider can give you a PUK code. Ring them up and give the details and they will give you a code. The service provider is who you have the telephone account with.

Sorry to tell you, Verizon strictly forbids adding non-Verizon Wireless phones on their network. You're stuck with Tracphone or other GSM pay-as-you-go providers.

I assume you're talking about the Optimus G? If not, please provide specific model number. Anyway, it's been awhile since I've had that phone, but when I did, you had to have a computer to root it. I don't believe that has changed. Step-by-step instructions can be found at the following links:

Try taking out the battery and the sim card, then put back in. After powering phone back up, it you should be able to make non-911 calls. Worked for me. Godd luck!
